Missed periods
LMP date is 15 june. Prolactin is normal but TSH is 5.310 which was taken after breakfast. If I take medroxyprogesterone, withdrawal bleeding comes , then followed by normal period at 30th day next month, then followed by delay next month.

A TSH in that range can affect cycle regularity and may be part of the reason for missed periods and the need for withdrawal bleeds. Optimising thyroid levels often helps cycles settle on their own.
Next Steps
Please see your gynaecologist with these reports so thyroid management and cycle regulation are planned together, not one without the other.
Health Tips
Recheck TSH fasting, first thing in the morning, for a more accurate value.
