Menstrual is not proper

Asked for Female, 24 Years

Ovaries are enlarged & show multiple follicles measuring < 10 mm. right having 4.08 x 2.07 x 2.60 cm (vol 11.46 ml) and left having 3.71 x 2.33 x 3.00 (vol 13.61 ml) Also, my menstrual is not proper and not ontime. Is it normal OR do I have do surgery?
