Hello, Thankyou for reaching out! The combination of Azee 250 (Azithromycin), Allegra 180 (Fexofenadine), and Meftal 500 (Mefenamic acid) is not known to cause serotonin syndrome. Serotonin syndrome usually occurs due to drugs that significantly increase serotonin levels- some classes of antidepressants and serotonergic agents like tramadol or linezolid. None of the mentioned medications are serotonergic. However, if your sister experiences symptoms like confusion, tremors, rapid heart rate, or fever, seek immediate medical care as a general safety measure.
