Medical Certificate for Ear Pain

Asked for Male, 26 Years

My both ears are paining from past 4 weeks. I have to use headphones for long hours in my work profile. When I'm using headphones or direct mobile, ear pain is resulting in headache and neck pain also. I have requested to my boss for early relieving from the job. They asked me to get a medical certificate for it. Kindly help me.
