Medical certificate is a legal document and can be provided only if you are ill and cannot attend your place of work due to illness or others are at risk of contacting the same infection as you. It cannot be given for any other reason.

Its ridiculous to ask for a medical certificate here. On an online medium. A medical certificate is a legal document. Go to your nearest doctor and seek one.
