MDD + OCD + SAD
For MDD + OCD + SAD If SSRI is giving partial results (for a hypertensive and pre-diabetic) and all SSRIs are tried with maximum doses and still having partial results for OCD If I cannot tolerate SNRI or NDRI augmentation because of risk of hypertension If the patient had an episode of schizophrenia 20years ago. What can I argument with? Antipsychotics? Is this a long term tolerable option? Is Respiridone a good choice? Clomipramine another option? Thank you

You can augment SSRIs with psychotherapy ( CBT). If antipsychotic drugs are to be used you should use one with least metabolic side effects like aripiprazole. Also not to forget about lifestyle modifications, which are going to prevent you from landing into diabetic state.
