Given your father had diabetes, you are at increase risk of diabetes. Given that your father had kidney failure due to diabetes, you will also be at increase risk of kidney disease if you get diabetes.
You are very young, so the anticipated lifetime risk of kidney disease is not insignificant. Though you might be all fit and able to donate , looking for a alternate donor is what I advice before going ahead being yourself as donor

You will be evaluated thoroughly before being considered fit to be donor. Post surgery, you will need to be on regular follow up and to continue precautions considering family history. To get ESRD as you asked will be rare if you are cleared to be donor after thorough evaluation.
