It looks like what we call Oral Hairy Leukoplakia
Do the following:
1. Blood tests - HbA1C, HIV, MonoSpot test, Hepatitis B and Hepatitis C
2. Five days of oral nystatin drops

It's not thrush.
Don't worry at all as u r on.pep very early continue it. Pep gives u protection. If u see toung it gives u anxiety. It will go slowly. Nothing to worry.
You can go with
Folic acid5 two times after food for 2 weeks.
Locally apply zytee lotion two times before meals apply with ear bud..two weeks.
Based on the photo and history, this does NOT look like oral thrush. Oral thrush usually appears as thick, creamy white patches that can be scraped off and leave a red/raw surface underneath. Your tongue shows a diffuse coating and surface dryness, which is more consistent with benign tongue coating, dehydration, anxiety, or medication-related changes. At 10 days post-exposure, HIV-related oral thrush is extremely unlikely, especially since you started PEP within 12 hours, which is highly protective.
Next Steps
• Continue PEP exactly as prescribed and do not miss doses
• Maintain good oral hydration; drink plenty of water
• Gently clean the tongue once daily (no aggressive scraping)
• Use plain mouth rinses (salt water); avoid antiseptic overuse
• Lip dryness can be managed with a simple moisturizer or petroleum jelly
• Proceed with routine HIV testing as per PEP protocol for reassurance

These tongue changes and dry lips are not suggestive of HIV. You started PEP within 12 hours, which is excellent and highly protective. At 10 days post-exposure, HIV symptoms are extremely unlikely. The findings are more consistent with PEP side effects, stress/anxiety, dehydration, or a benign tongue coating, all of which are common.
