Is 2nd Biopsy required?
As per PET-CT scan the report, enlarged lymph nodes/nodal masses were found atleast at 5 places. CT guide FNAC from left para-aortic lymphadenopathy. As per microscopy of biopsy, 1. FNAC smears are haemorrhagic & sparsely cellular & show scattered atypical cells which are intermediate to large in size & show highly N:C ratio, irregular nuclear borders, coarse chromatin and scant cytoplasm. Atypical mitotic figures are seen. Crush artefact is noted. 2. The sections studied shows blood clot with entangled atypical lymphoid cells exhibiting hyperchromatic nuclei, irregular nuclear margin and scant cytoplasm. Brisk mitoses are identified. According to the above report please let us know the cancer status and whether another biopsy is required?

Excisional biopsy from supraclavicular lymphnode will be easy and likely most conclusive.

Imaging wise, this looks like a lymphoma. Biopsy and IHC is required. As per PETCT there are neck lymph nodes. Excision biopsy of this lymph nodes is to be done and send for histopathology + IHC. Once lymphoma and the type is certain, we can discuss treatment options.

Yes ,the biopsy is required. But should try from the neck node. After the biopsy report ,IHC is also required for definite diagnosis. You can consult over phone for further queries.
