🔬 Test Results:
Serum Iron: 18.8 µg/dL → Low
UIBC (Unsaturated Iron Binding Capacity): 8602.1 µg/dL → High
TIBC (Total Iron Binding Capacity): 8602.9 µg/dL → High
TSI (Transferrin Saturation Index): 0.2% → Very Low
Hemoglobin (Hb): 9.9 g/dL → Mild anemia
These values strongly indicate iron deficiency anemia (IDA):
Low serum iron + low TSI
Elevated TIBC/UIBC (transferrin not saturated due to iron deficiency)
Mild anemia (Hb < 12 g/dL for females)
🩺 What Should Be Done – As per Guidelines:
1. Identify Cause of Iron Deficiency
Check for blood loss (e.g., heavy menstrual bleeding, GI bleeding)
Assess dietary intake and malabsorption causes (celiac disease, parasitic infection, etc.)
2. Start Iron Replacement Therapy
Oral iron salts (e.g., ferrous sulfate 200 mg TDS or equivalent elemental iron ~60–100 mg/day)
Best taken on empty stomach or with vitamin C for absorption
If not tolerated or ineffective → consider IV iron
3. Monitor Response
Reticulocyte count rise in 7–10 days
Hb increase by ~1 g/dL every 2–3 weeks
Continue iron for 3–6 months after Hb normalization to replenish stores
4. Dietary Advice
Increase intake of:
Red meat, liver
Green leafy vegetables
Iron-fortified cereals
Citrus fruits (for vitamin C)
5. Follow-up Investigations (if no improvement):
Stool for occult blood
Endoscopy/colonoscopy if GI blood loss suspected
Evaluate for chronic illness if iron studies remain abnormal
⚠️ Red Flags to Urgently Investigate:
Persistent low hemoglobin despite therapy
Signs of occult bleeding
Weight loss, fatigue, or chronic symptoms without known cause
