Sitting in warm water hot enough to take a bath twice daily may make you comfortable but please don’t ignore the advice of surgeon because Ayurvedic treatment is not sufficient where surgery is required you may take a second opinion from another surgeon if in doubt.

Happy to know that you are able to tell internal anal sphincter pain. If pain is prevailing inspite of medication, diet and lifestyle modifications, and is disturbing your day to day routine inspite of active treatment, you will be a candidate for surgery.. If pain becomes well, keep a watch.
Next Steps
diet and lifestyle changes

Best treatment I suggest is to visit a surgeon. 90% of fissure in Ano can be treated without surgery. Sitz Bath is a must to prevent infection and control pain
