Look out for any danger signs of head injury like repeated vomiting, unconsciousness, not behaving in normal way , seizures, bleeding or water leaking from ear, nose etc Fever is most likely unrelated to the fall event, but keep a close watch Any red flags , then visit hospital immediately

If no warning signs like loss of consciousness or altered behaviour/ drowsy / blurred vision/ slurred speech, and the wound is healed completely with no swelling, you can wait and give paracetamol. But if still fever is persisting or accompanied with any other symptoms please do consult nearby pediatrician.
