Hi
Your concern is your wife gets violent when she has a migraine headache. It is necessary to know what is causing migraine. It is necessary to know if she is stressed or anxious about something. It is known that chronic stress can cause migraine. It seems like chronic stress could be triggering migraine and migraine could be triggering anger. It is also necessary to know if your wife has physical health issue and if she is on medication. In order to reduce frequency of violent behaviour the root cause of migraine has to be determined. From what you have mentioned it seems like the impulsive behaviour is severe. Your wife will require a combination of medication and psychotherapy.

Hi... This may be more than just migraine. Please consult a Psychiatrist. These bouts of anger may be a sign of an underlying condition. Such conditions can be addressed with a combination of medication and psychotherapy. Also, do mention age and a brief background of the patient. It helps in making an appropriate assessment of their situation.
Next Steps
Consult a Psychiatrist first and a Psychologist later.
