Hypothyroidism Myxedema = severe hypothyroidism SX – weakness/fatigue, feel cold, dec memory, constipation, weight gain, depression, bradycardia, dec DTRs PE – all (look for weight gain, dry skin, loss of hair, macroglossia, goiter, non-pitting edema (myxedema)
Next Steps
DX – TSH (increased), Free T4 (decreased) Antibodies against thyroid peroxidase (positive in Hashimoto’s Thyroiditis) • CBC, CMP, fasting lipid panel
Health Tips
TX – maintain a euthyroid state, w/ F/U Q6 wks • Synthroid (levothyroxine), start 12.5–50 mcg QD, then 50-200 mcg QD • 2/3 the dose in elderly
