I have been recommended hydroxyl mouthwash for multiple issues, but am i supposed to dilute it water or not??

You don't have to dilute it. Take the mouthwash in the quantity as advised by your doctor and put it in your mouth. Swish for 30 seconds to 1 minute and spit it. Do not swallow.
