An endocrinologist is the right person to start and monitor feminizing HRT; a gender-affirming physician or psychiatrist assessment usually runs alongside, not instead. Standard baseline work-up before starting: CBC, LFT, KFT, fasting sugar and lipids, testosterone, estradiol, LH/FSH, prolactin, TSH, and blood pressure with weight. Prolactin and liver tests matter because estrogen and anti-androgens act on both. After starting, levels and labs are rechecked at 3 months, then 6 monthly.
