How to control fever
From last night she has a fever. Hope so OPD are open from yesterday. Just wanted a suggestion for the night.

For fever for a 4 year old girl, Best possible options are: (1) Tepid sponging using Luke warm water (Slightly above the body temperature), to make sure that there is no fan running in the room directly above the child (As evaporation and further reduction of Temperature of water may cause discomfort and shivering for the child, resulting in sometimes persistent fever, or increase in fever). Rule is to wipe the child mainly around groin areas and underarms and then rest of the body and to make sure to wipe off the same water using dry cloth with few seconds, and repeat the same procedure for about 5 to 10 minutes till the temperature drops. Kindly use tepid sponging only if temperature is above 101.5 F and not coming down her regular dose of Paracetamol. (2) For temperature below 101.5 F and above or equal to 99.6 F (when checked underarm and when the child is on single thin layer clothing/ not when she is covered with multiple layers of clothings & if she has been kept wrapped in Multiple clothings, to make sure she is kept back in single layered clothing for atleast 5 min before checking the next temperature level). (3) In case she has low grade fever (Less than 101.5 F and more than 99.6 F) spikes, but repeated ones with a gap less than 6 hours, then to try tepid sponging for some time to get that time gap in order to get closer to 6 hours gap for next dose of Syrup Paracetamol. (4) Also, make sure that you have already booked an online appointment for seeing your Pediatrician so as to make sure that once this phase is tackled by these measures, you have a definitive opinion from your Pediatrician as early as possible. (5) Watch out for other possible associated symptoms like - (a) Thirst irritation, throat pain, reduced solid food intake (even during afebrile episodes, in between fever spikes when she is relatively active); (b) OR sometimes to notice if her activity is poor even in between fever spikes (which may need some more time for the Pediatrician to figure out the cause and possibly order a few tests too); (c) Any other unusual Symptoms not normally seen, (d) Her orientation and lethargy, (e) Burning while urinating/ increased urination, (f) Altered behaviour/ mental status/ excessive sleepiness, (g) Tummy pain, loose stools, vomiting, nausea, etc. To make sure that these points are observed for and mentioned to the Pediatrician as soon as you meet him/her at the next available appointment for the same.

Sponging is the simplest way to decrease fever.. If the fever is not so high try sponging if it is high give paracetamol Syp .
