A very high ESR usually has an obvious cause, such as a severe infection, marked by an increase in globulins, polymyalgia rheumatica or temporal arteritis. The low vitamin D levels should be corrected as it can accelerate the osteoporosis. I would advise you to meet an orthopedician so that you can be examined and undergo investigations such as RA factor, X-Ray C-Spine, etc. to find the underlying cause and have it treated accordingly.
