My husband had a heart block, he has stent placement. He doesn't have cholesterol, BP, sugar and no smoking, no drinking habits, no heredity. What could be the reason for heart block?

The reasons you have mentioned are the commonest cause of a atherosclerotic coronary artery disease.
But there are hidden factors which can contribute to accelerated atherosclerosis in young age.
Stress
Diet
Sedentary habit
BMI
Apolipoprotein
Post Covid We are seeing some early disease
And many genetic factors which medical science is yet to establish as the cause
