Hair loss problem

I am 23 years old and I am suffering from heavy hair loss from past 3 years .I have tried tablets,oils,shampoo ,minoxidil but nothing really worked !!! The skin of my head(Scalp) continuously get dried and the whole dead layer falls in the form of big yellowish fragments !!! Please suggest the best treatment
