Doctor, my husband (40 age settled in UAE)is currently on day 9 of his 3rd H. pylori treatment (Amoxicillin + Levofloxacin + PPI). His first treatment was Amoxicillin + Ciprofloxacin + PPI, and the second was Metronidazole + Tetracycline + Bismuth + PPI, but both were unsuccessful. Endoscopy showed mild antral gastritis with complete intestinal metaplasia in the stomach and short-segment Barrett’s with incomplete intestinal metaplasia in the esophagus (no dysplasia). H. pylori test was positive after the previous treatments. He is experiencing gas, acidity, and bloating. Should we be concerned about treatment failure, and what is the next best step?

H.Pylori infection requires long term treatment. Treatment failure is common, more so when treatment remains incomplete. Also, lifestyle modifications need to be followed strictly:
1.Timely meals & sleep
2.Avoiding: oily spicy non veg food and all kinds of addictions (tea, coffee, smoking, alcohol......)
3.High Fibre diet
4.Plenty of oral fluids 3-4L/day
