Hi. Right combination of Glaucoma medications is very important. Nowadays, highly effective medicines are available. So, if your pressure is not controlled by medicines, it may be due to wrong dosage, wrong combination  or failure of medical treatment in treatment in glaucoma. Failure calls for next procedure which include surgery etc. Iridotomy and glaucoma surgery are two different procedures, with different indications. How many medications you are using for pressure control.??
