Genital pigmentation, itching, pain tip.
I had itching and a ringworm infection in the private area earlier, which was treated and resolved. It has now returned. For the past 2 years, I've had skin pigmentation changes on the penis and surrounding areas, and recently this has started itching again, along with pain at the tip when touched, especially when exposed. My foreskin is only partially retractable when dry. I've also noticed reduced firmness and size during erection. I recently checked my blood sugar and it was normal. I'd like to know if the dark pigmentation is due to hormonal changes, what is actually causing all this, and whether it needs medication or will resolve on its own

Aapke symptoms recurrent balanitis/fungal infection ke saath tight foreskin (phimosis) ki possibility suggest karte hain. Dark pigmentation zaroori nahi ki hormonal problem ki wajah se ho; previous inflammation/infection ke baad pigmentation reh sakti hai. Erection mein reduced firmness pain, inflammation, anxiety ya other factors ki wajah se bhi ho sakti hai. Clotrimazole 1% cream – affected area par thin layer BD × 2 weeks. Area ko clean aur completely dry rakhein.
Next Steps
Foreskin ko forcefully retract na karein. Agar tightness persist karti hai, urologist/dermatologist examination karwayein; non-scarring phimosis mein doctor-supervised topical steroid with gentle stretching 4–8 weeks ek treatment option ho sakta hai. Agar infection baar-baar hota hai to HbA1c/fasting glucose bhi check karwana useful rahega, even if one recent sugar reading normal thi.
