Calculus of size more than 10mm requires surgery. However if he is not having any type of stone related symptoms like pain in right upper abdomen, abdominal bloating, gastritis, he can be managed conservatively. But keep in mind of the requirement of surgery. As of now not required as fever episode is going on. So please wait for now till this episode resolves and future course depends on his symptoms.
Next Steps
get treated for this fever and hypertension and get discharged.
Health Tips
lap cholecystectomy is safe procedure keep low threshold for surgery if he has any kind of complaint in abdomen.

Fever is also a form of symptoms in Acute Cholecystitis..
If he had cardiac issues for anaesthesia concern..better postpone it
As long as stone in bladder it will be painless but if it moves to duct..in your language to mouth of gall bladder..it causes pain
But with out warning symptoms there may be silent complications so better get examined..Take pros and cons and then go for surgery or conservative management
Next Steps
Consult General surgeon
Health Tips
Take medication for pain..
Take foods with minimal oil content and Take as small intermittent foods than taking at time normal diet
At 69yrs, acute episode of cholecystitis, should be managed on medications.(Gall bladder becomes swollen and inflamed.)
If he suffers from any heart related issues, do not think about surgery.
The pain can occur again because of stone and inflammation.
If the surgeon you are admitted under says to proceed for surgery, ask him if you can wait for 3-4 weeks.(inflammation subsides during this period)
If symptoms continue, then get operated.(Laparoscopically)
The 13.4 mm stone cannot be removed solely, Gall bladder has to be removed.
Next Steps
Manage acute episode on medicines. If symptoms occur after 3-4 weeks, go for surgery.
Health Tips
Risks of surgery to be discussed with surgeon and anaesthetist.
If the patient is having fever..and presently there are no symptoms of pain upper abdomen and Vommiting..it means Gall bladder stone is an incidental finding..so it is not required to undergo gall bladder surgery as of now keeping in mind age of the patient..
Secondly gall bladder are seldom treated by medications..u will have to undergo surgery for same is near future..till then again put the patient on short and frequent meals..and avoid being full stomach..and avoid oily and spicy food..!!
