You need investigation to find out the reason for the swelling
Get her Hemoglobin n blood pressure checked
start a good protein rich diet
get an ultrasound done to ensure fetal well being

Swelling of hands and legs since when ? Was she percieving baby movements prior to this because at 5 months you didn't really perceive very well
Also please consult a gynec , need to evaluate for BP as well
