What you describe sounds like panic attacks with anticipatory anxiety, and the gut symptoms of IBS very commonly go hand in hand with it, each feeding the other. The fear of collapsing or needing hospital during an attack is a classic and very treatable part of panic disorder, and premenstrual worsening is also well recognised. Importantly, panic attacks feel frightening but are not dangerous.

Please arrange a proper psychiatric assessment; cognitive behavioural therapy for panic works very well and medication, if needed, should be reviewed and adjusted by a doctor rather than carried around as a rescue tablet. Do not stop or change any prescribed medicine on your own. Alongside, practise slow diaphragmatic breathing during an attack, keep regular sleep and meal timings, cut down caffeine, and do some daily physical activity. A basic check with haemoglobin, thyroid profile and vitamin B12 and D is reasonable to exclude physical contributors.
