Hi dear
External piles and fistula are two different things .
While external piles if not causing any complaints can be left alone .
Fistula can cause issue later on and many people suggest to get it treated /operated .
It depends upon the size and location of fistula so its better that u go to a general surgeon and get urself examined for further guidance.
