Hello. I understand your complain of pain while chewing. As per the scan and you given history , the pain is mostly due to combination of endo perio lesion + caries + multiple fine crack lines. It is definitely treatable. Kindly visit nearby Dentist
Next Steps
Visit Dentist. Avoid chewing hard food. Avoid hot or cold pack massage. Also avoid too hot or too cold beverages.
Health Tips
Avoid delaying visit as it might lead to formation of abscess(pus) ; avoid taking painkillers for long term to get rid of pain instead go for the required treatment after clinical assessment of the tooth

Based on the CBCT findings, the upper left second molar (27) appears to have features suggestive of an endo-perio lesion with associated bone loss and possible crack lines. This is not considered a normal finding and may explain the pain while chewing. However, the exact diagnosis and prognosis depend on a clinical examination, vitality testing, periodontal assessment, and correlation with your symptoms.
Next Steps
I would advise you to consult an Endodontist/Periodontist at the earliest for a detailed evaluation. Avoid chewing hard foods on that side until the tooth is assessed. Early treatment can help prevent further progression and improve the chances of saving the tooth.

Hello .I understand your concern. And acc. to the cbct report of all the 3 teeth i.e. 26,27,28.All of them have problem which needs immediate treatment.the crack lines found on the teeth can increase and aggravate if you bite on hard items so avoid doing that from left jaw and there are no home remedies for this particular dental problem maximum you can take an analgesic but that will not cure the infection which is already involving the root canal and the tissues around the teeth root ( gums, cementum and periodontium)which is why this disease is called endo perio lesion.
Next Steps
you should definitely consult a nearest endodontist/ periodontist and get the needful treatment done.
Health Tips
avoid biting/ chewing on hard food items from the affected side of jaw.
