In my 20th week anomaly scan, there was an isolated EIF in the right ventricle. My NT and double marker have come back with Low risk. Need we be worried here? What would be the first step?

Isolated echogenic foci in the right ventricle is not of much significance unless associated with any other soft marker or anomaly on the scan. But still to be on the safer side, you can get a fetal echo done at around 24-26 weeks.
Echogenic intracardiac foci is a normal variant if it is isolated need not worry about that .. suggested fetal echo scan at 24 weeks of gestation..

Usually of it is isolated findings then it is considered as a normal finding provided, all the related genetic markers are normal. Follow up after 4 to 6 week four fitel, echo is needed
Do you have any risk factors?
Diabetes,heart disease
Any previous child with mental retardation or heart disease?
Family history of heart disease(congenital)
Are there any other anomalies in the scan?
If no to all these questions then no need for further tests .
If yes then there may be 4 to 5 percent chances of dien syndrome or patau syndrome
