Dull upper abdomen pain
Hello Doctor, I am a 36-year-old male with Grade I fatty liver (improved from Grade II). My recent LFT is normal (AST 22.9, ALT 19.6, bilirubin and albumin normal). Platelet count is 179,000. However, I still have a mild dull pain (2–3/10) in the right upper abdomen, sometimes on the left, along with gas and occasional soft stools. Could this pain still be related to my liver, or is it more likely due to a stomach or bowel problem? What would you recommend as the next step?

Fatty liver can cause mild right upper abdominal pain but in this case we should keep other differentials. Do you have weight loss? Do you have black stool? Does this pain varies in relation with meal? Other possible differentials are Gastritis, EPS and CP.

Look for alaram features like weight loss, black stool, relationship of pain with foods. Do a CBC and Upper GI endoscopy + RUT for H. Pylori

Fatty liver is a painless problem: hence this pain is more likely to be due to digestive system related to colon (large bowel). A stool exam will help. Otherwise a course of Intestinal antibiotics should help
