Any Allopathic doctor with MBBS can treat any disease if he feels that he is competent to treat that disease. The problem starts if the patient complains that he has been harmed or if a patient died. Then the doctor will have to prove that he has got some training to tackle those patients. The quantum of punishment is also different for those who hold the requisite qualifications (because in that case the doctor is tried for error of judgement) and for those who don't hold requisite qualifications (they are tried for "murder"). A doctor holding a degree from any other stream of Medicine is governed by AYUSH Rules. They have never been tried for Medical negligence so far .
