You need to adjust your insulin regimen, periodic dose adjustment of insulin required. Need to optimise your non insulin medicines. Diet and exercise protocol is also very important for you. Consult doctor to have a prescription.

You are taking basal bolts regimen, which is considered the most physiological way to take insulin. The insulin dose needs to be adjusted till you reach your target glucose, ie, FBS 100, PP 140. Sometimes insulin injection technique may be wrong wherein insulin may not be reaching your circulation

For any diabetic patient to control his sugars most important thing is diet and exercise. So first ensure about lifestyle changes. Using only short acting insulin( Novo rapid) will not control the sugar unless it is used with long acting insulin. So with these two things in mind we can control sugars much better
