Dandruff, itching, heavy hairball, patch

My daughter 10yr old past 2yr dandruff continuous, KZ shampoo used as per Dr advice, no use, come back again, hairfall, patches...fyi pic attached. Please Advise.
If I use cloritamazole cream it goes for 10days but repeats again.
