CKD ,diabetic ,CABG anemi
Patient is having serum creatinine 5.93 and GFR at 8.9.Currently admitted in hospital as the patient has following discomforts 1) swelling in feet, ankles,hand , face 2)shortness of breath 3) general fatigue weakness 4)nausea 5)muscle cramps , hand tremor He is being treated to dehydrate but creatinine is not coming down (5.83 to 5.88 to 5.93) though urine output is more than input and body weight is also reducing. Is dialysis the only next step or still there are some non dialysis options to explore?

Hi , You need to consult nephrologist. With such a background kidney puts a ceiling on amount of urine that can be excreted. Liklehood of creatinine coming down depends on whether it is acute or chronic kidney disease. If chronic it may show minor variation but usually goes up with diuretic which is essential. Ultimately dialysis may be required sooner rather than later . Your treating nephrologist will take the call when to dialyse. There is no alternative yet to dialysis. Kidney transplant for suitable candidates is the final answer.
