My father is ckd patient, his criatinin is 4.42 ,uria is 120 and based on Renal Scan relative functionality of left kidney is 10% and right kidney is 90%. How to decrease or maintain creatinin level?

Once he is already a known ckd, and creatinine is 4.4 reducing it is almost not possible unless it's a recent worsening..... Pls furnish his complete reports. If the creatinine worsens he may need dialysis

You didn’t mention comorbidities like High BP or diabetes, if your father is having them, they need to be controlled.
Ultrasound scan does it show any stones or prostate problem?..need to be addressed accordingly
If there is any recent insult to the kidney, it might recover to its baseline orelse it will not
Advice:CKD diet( low protein, low salt diet, fluid intake 1 litres per day. avoid fruits, coconut water, phosphorus rich foods.)
Medication adherence
Vaccination
Av fistula creation
Review monthly with nephrologist with KFT and serum electrolytes
Be aware of warning signs when dialysis needs to be initiated
