As you are having infection and bilateral inguinal lymphadenopathy you need to take antibiotics for few days and treatment for anal fissure and if possible get yourself examined by a doctor , after proper treatment look for the size of lymph nodes then only any further investigation to be done

For the anal fissure - *Take a stool softener/laxative as advised to keep the stools soft. * Apply local lignocaine ointment before passing stools to reduce pain. * Take a sitz bath 2–3 times a day: sit in a basin or tub containing warm water for about 10–15 minutes each time for 5 days. To prevent recurrence: * Follow a high-fibre diet, including plenty of fruits, vegetables, and green leafy vegetables. * Drink 3–3.5 litres of water daily (unless you have been advised to restrict fluids for another medical condition). * Include curd/yogurt in your daily diet, as it may help maintain healthy bowel habits. * Avoid straining during bowel movements and try not to delay the urge to pass stools.
Next Steps
Your Ultrasound shows inguinal lymph node enlargement. Which needs clinical examination by a surgeon.
