It does not look like decay but staining of teeth. You can get her teeth cleaned and regular check ups every 6 months should be done to detect any caries

Firstly there is no premolar in children.
Her teeths look dirty and full of plague.
Cavity in the teeth can be filled after a checkup.
21 months is too young for cavity.
You need to take care of your baby's hygiene.
Teach her how to brush or you do it till she learns.
Use pea sized fluoridated tooth paste for brushing.
Donot give milk over night.
Brush her teeth a twice daily and clean the tongue.
Hello,
It doesn't look like caries and just need some preventive treatment. If the child is not brushing then you have to brush her teeth with a finger brush and a non flouridated toothpaste.
Next Steps
Make sure that you brush her teeth at night it is more importatnt than brushing in the morning.
Hello,
Not all black spot is Decay.
As shown in pic it doesn't seem like to be.
make your daughter to clean her teeth properly under your observation, everyday.
Using a brush is more important than paste. Use minimum amount of paste. Be a role model, you should brush in front of her so that she follows. Consult a dentist immediately for assessment. If she has decay, exposure to sweets and sugar should be restricted. Don't give sweetened milk especially at night. By the way there are no premolars in a 21 month old child.
Next Steps
Consult a dentist immediately for assessment. If available a pedodontist (specialist in child dentistry).
Health Tips
Does she sleep with her mouth open while sleeping? If so mention that to the dentist or consult ENT doctor to rule out nasal obstruction (blocked nose).
