Dear sir.
As I can understand, you are suffering from diabetes, hypertension and had undergone a bypass surgery. And in the last 15 days your BP shot up thrice.
The first thing that you must do is to visit a qualified cardiologist as early as possible and get yourself personally examined with BP check & show him all the medicines so that your medicines can be optimized properly. Doing any change of medicine on your own without guidance of a cardiologist will be harmful.
