  4. Blood n stool. Become little harder.

My problem is i have pain during bowel movements. For every 3 to 4 months my stool comes with blood and pain. After that my anus hole area become little harder.

It is anal fissure.  If the fissure is acute,  medical treatment will gives complete cure. If it is chronic,  success rate with medical treatment is around 50 ./. Need clinical examination.
