All depends on the cause of raised creatinine. Whether due to acute(short term) or chronic diseases(long term). Hypertension, diabetes, acute or chronic infection in kidney, drugs side effect, obstruction in the urinary passage are the common causes. Consult a nephrologist to delineate the cause and treatment is planned as per cause and effects. Dialysis is only a supportive treatment and does not treat the cause. Transplant is advised only in advanced stages of chronic kidney diseases.
