Since your wife is 4 weeks pregnant, bleeding is usually common it is called as implantation bleed. And ultrasound report says early intrauterine pregnancy, closed os; so there is nothing much to worry about. Let’s wait for a week and repeat scan.
Next Steps
Repeat scan after a week. Confirm fetal pole and cardiac activity in further scan.
Health Tips
If bleeding continues, there is sudden gush of blood or if signs of pregnancy like nausea suddenly disappears then do consult a gynaecologist and do not avoid these symptoms. Till then just take good rest, avoid any strenuous physical activity and have good healthy food. Continue folic acid tablets.

I have gone through the report. It shows an early intrauterine pregnancy with a small peri-gestational collection and no fetal pole seen yet, which can be normal at 4 weeks. The right ovarian corpus luteal cyst is also a common finding in early pregnancy.
Next Steps
At this stage: Continue adequate rest and avoid exertion. Continue folic acid as prescribed. Avoid intercourse until cleared by your gynecologist. Most importantly, please follow up with your gynecologist at the earliest for clinical examination and repeat scan in 1–2 weeks to check for fetal pole and cardiac activity.

Sir your wife is having complaints of discharge and bleeding along with lower abdomen and back pain. TVS Scan has been done and the report says gestational sac is present but to confirm pregnancy they have advised to get TVS Scan after  1 week. Mild spotting or only mild bleeding is okay but since ur wife is having lower abdominal pain and back pain it is necessary to get evaluated by a Gynecologist sir . Also would like to know the blood group of your wife sir.
Next Steps
Consult a Gynecologist as soon as possible if bleeding and pain present sir.Need to get Tvs or b-hcg test if Gynecologist advises . TVS Scan as advised 1 week after.
Health Tips
If heavy bleeding with abdomen and back pain  go immediately to Emergency dept or Consult a Gynecologist in person as soon as possible

Hello, thank you for sharing the details. Bleeding and discharge along with lower abdominal and back pain at 4 weeks of pregnancy is not normal and needs urgent evaluation. It could be an early pregnancy complication like threatened miscarriage or ectopic pregnancy. Please visit a gynecologist as soon as possible with the TVS report. They may do repeat scans and blood tests to check the pregnancy. Meanwhile: Avoid heavy activity or lifting. Note the amount and type of bleeding. Seek immediate help if pain worsens, bleeding is heavy, or she feels dizzy or faint. Early assessment is important for her safety and the pregnancy.
