Biliary Gastritis & Anemia
Patient Profile & Symptoms: Shammi Aktar, a 19-year-old female, presents with severe abdominal burning, a rock-like heaviness immediately after eating, and difficulty breathing when lying flat after meals. She experiences severe nausea and stomach pain from oral iron tablets. She also suffers from chronic heavy menstrual cycles (menorrhagia). Key Diagnostic Findings: Upper GI Endoscopy: Confirms Biliary Gastritis with raw, red, deeply inflamed tissue (hyperaemic mucosa) in the lower stomach. H. pylori bacterial test is negative. Abdominal Ultrasound: Confirms a normal gallbladder with zero stones or blockages, but reveals a thickened endometrium (uterine lining) which causes her heavy periods. Hematology Blood Report: Confirms Microcytic Anemia with a low Hemoglobin of 10.9 g/dL, low MCV (76.3 fL), and low MCH (26.7 pg), showing small, pale red blood cells due to chronic iron loss. Kidney and liver functions are completely healthy.

A thickened uterine lining explains your heavy periods, and that monthly blood loss is very likely behind the low haemoglobin and the small, pale red cells on your blood report. The stomach burning and heaviness after meals point to the biliary gastritis your endoscopy already found, which is a separate problem from the heavy bleeding.
Next Steps
Treating the heavy periods through your gynaecologist is what will actually correct the anaemia long term, since tablets alone can't keep up with ongoing blood loss. For the stomach symptoms and trouble tolerating iron tablets, your gastroenterologist can suggest a gentler option or a different route.
Health Tips
Please see your gynaecologist and gastroenterologist together, since treating only one side will leave the other untreated.
