Hii.. I can completely understand what you're going through. Understand that OCD is treatable with proper management and therapy.  The symptoms like unwanted sexual urges regarding fms is really distressing. If there is persistence of symptoms then dose adjustment might be required with proper guidance.
Next Steps
You may continue the same dose (40 mg) using available brands if possible. Increasing to 60 mg should only be done under medical supervision, as dose changes need monitoring for side effects and response. If affordability is an issue, consider government hospitals or low-cost psychiatric services for proper guidance.
Health Tips
Do not try to suppress or fight the thoughts—they tend to increase • Remind yourself “these are OCD thoughts, not reality” • Keep yourself engaged in activities • Practice relaxation techniques • CBT with ERP is very effective for OCD and can help significantly OCD is a treatable condition, and with the right dose and therapy, these distressing thoughts can be brought under good control.
