Your reports suggest vitamin B12 and folate deficiency, which can cause tingling, neuropathy, and may contribute to low libido. Get treated promptly with B12 (often injections initially) and folic acid. You can consult me for a detailed evaluation of your low libido and ejaculation-related symptoms, along with appropriate treatment.
Your reports and symptoms strongly suggest a clinically significant vitamin deficiency, not just a borderline blood test.
The important findings are:
Vitamin B12: 226 pg/mL (higher than before, but still relatively low for someone with neurological symptoms).
Methylmalonic acid (MMA): 481 â elevated, indicating functional B12 deficiency.
Homocysteine: >50 â markedly elevated, which can occur with B12 and/or folate deficiency.
Folate: 1.19 â significantly low and requires correction.
Ferritin: 32.3 â on the lower side, depending on the laboratory reference range.
Your symptoms of:
Pins and needles in the hands and feet.
Floating sensation/dizziness.
Reduced libido.
Intermittent sharp pain after ejaculation.
are compatible with vitamin B12 and folate deficiency affecting the nervous system, although they are not specific and other neurological conditions may also need to be excluded.

Health Tips
Take the prescribed treatment consistently and avoid stopping it early once you begin to feel better. Recovery of nerve-related symptoms can take weeks to months, and improvement is often gradual. Eat a balanced diet containing foods rich in vitamin B12, folate, and iron (or appropriate alternatives if you follow a vegetarian diet). Seek urgent medical attention if you develop: Progressive weakness in your arms or legs. Difficulty walking. Loss of bladder or bowel control. Vision changes or confusion. The good news is that many people with B12 and folate deficiency improve significantly once the deficiencies are adequately corrected, although symptoms that have been present for a long time may take longer to recover. Thank you
Consult a Hematologist or Internal Medicine physician to determine the cause of persistent B12 and folate deficiency.
These all the symptoms you have like pinning sensation it is all bcoz of B12 deficiency. So need to proper evaluate its deficiency you have to consult the Hematologist.
