Hi there, I am really sorry that you are in so much pain and discomfort. I hope my answer below may help you and ease your suffering. May god bless you. Ashwani Singh. AS is a systemic chronic autoimmune spondyloarthropathy which affects large joints and Primarily Spine. It usually affects males more than females(4:1). It seems that you are affected by juvenile form of AS. Only 10% of juveniles with positive HLA - B27 show symptoms of AS. I am assuming that it has been diagnosed in your case as it has a diagnostic criteria, which includes bilateral sacroilitis, -/+ uveitis, HLA- B27 and other orthopaedic and systemic manifestations. It requires multidisciplinary approach. Treatment includes : 1) physical therapy like cardio respiratory exercises, swimming and others to maintain mobility and flexibility. 2) medical management like pain killers e.g NSAIDs, COX-2 inhibitors. These are first line therapies. Oral steroids are not recommended. -TNF-alpha-blocking agents- these are second choices and includes infliximab, etanercept, adalimumab and other drugs. Recent clinical studies have shown significant improvement in severity of symptoms with these but they are expensive and have many side effects. 3) Surgical in cases of established deformities or acute fractures or poor lifestyle issues or progressive neurological deficit.

If u have a confirmed diagnosis of ankylosing Spondylitis, then treatment will revolve around disease modifying drugs, steroids in the short-term along with Physiotherapy of all joints. You can either visit me at my clinic in Noida or any other Orthopaedic surgeon in your vicinity for starting the treatment.

In my opinion you must visit an Orthopedics and go for a thorough check up like Hlab27 blood test and x ray for pelvis and lumbar spine...to confirm AS...for stiffness u can go for physiotherapy sessions and hot fermentation at the clinic and at home also...followed by exs...like back extension exs strengthening exs...but only after pain subsides... get well soon take care...
