May be she is already unhappy about her looks  and very much disturbed about it. Above all ,  she is expected to.behave  properly with people .  That is to know to handle herself in communication, behaviour etc etc etc. ,  are highly irritating  her.
Next Steps
Do consult  a Psychologist  in ur city and give her space to.share her feelings and have clarity and then take up right decisions  for herself.

Respected Madam, I perfectly understand your concern for the well-being of your sister and applaud you for showing the concern as love for siblings is slowly waning over the past decade in our country. Now coming to your query, her anger issues and defiance to social views, emotional lability etc, could all be explained as a personality problem if it has been present for as long as you have known her. Hence the first diagnosis that I would like to consider or rule out in her case would be an Emotionally Unstable/Borderline Personality. On the other hand, if these problems are more of an acute onset, they could be part of an underlying mood disorder. Unfortunately the only way to distinguish between the two and to arrive at a proper conclusion regarding your sister would be an in-depth evaluation by a psychiatrist which is why I have no sure shot way of addressing her refusal to see a psychiatrist. That said, I am sure if you and your family try a bit harder, you could convince her to see a psychiatrist. Infact you can all go as a family and tell her its sort of a family therapy that she needs to be part of. If she cares for her family the same way that you do, I am sure you will be able to get her to agree. Another option is to search for psychiatrists in the area who are willing to do home visits. You also mentioned she is paranoid about her health which I am sure you can use to get her to a hospital and then maybe see a psychiatrist as a referral from the doctor she has agreed to see. The reason I am offering these options and insisting that she sees a psychiatrist is that whether its a personality problem or a mood disorder, both are going to persist and even increase if not adequately treated. So good luck to you and your family. Hope you are able to convince your sister and help her get better.
