Your gastroenterologist is actually correct — fluoxetine has evidence-based off-label use in chronic anal fissure. SSRIs like fluoxetine relax the internal anal sphincter, which helps heal the fissure by reducing spasm. This is a recognised treatment option when topical treatments haven't worked. It's separate from its antidepressant effect.
Next Steps
Take it as prescribed. Also use stool softeners (lactulose/isabgol) to prevent hard stools which worsen the fissure. Apply diltiazem or GTN gel topically if prescribed. Consult me on Practo if you have further doubts about your treatment.

U may have been confused. This.drug fluoxetine is an antidepressant and not used in fissure treatment. It in turn leads to constipation. Review with your Gastroenterologist about the same
