Hello Doctor, I recently had my blood tests and my Hemoglobin is 10.2 g/dL (reference range 12-15) and Serum Iron is 37 µg/dL (reference range 50-170), which appear to be low. My Vitamin D is also low at 23.6 ng/mL. Vitamin B12 is normal at 442 pg/mL. Could you please advise whether I should take an iron supplement? If yes, which type/dose would be appropriate for me, and for how long? I take meganeuron od plus occasionally and vitamin d supplement (60k) twice a month. I have sensitive stomach and suffer from IBS D. So which iron supplement will suit me? Please also let me know the best time to take iron and whether there are any foods/medicines I should avoid taking it with. Thank you

Yes, Hb 10.2 with low serum iron looks like iron deficiency anaemia, so you do need iron. With IBS-D, ferrous bisglycinate or iron polymaltose suits better than ferrous sulphate. Take one tablet daily or on alternate days, empty stomach or with light breakfast, with lemon water or amla. Keep tea, coffee, milk and calcium 2 hours away. Usually 3 months after Hb normalises to refill stores.

You can take iron, combined with folic acid supplements,  ,morning and evening, after meals, for 4 weeks.. Tab uprise 60,000 units once a week for 8 weeks.

Firstly I want to appreciate your clear way of expressing your problem. Secondly, you divide your MCV with RBC count. If it is less than 13 then you have a problem. Thirdly considering your anemia is due to iron deficiency and your having IBS D, the best iron supplement would be iron bisglycinate (chelated iron) or polysaccharide iron complex which are best tolerated. Take any good brand.

Your Hb is low and the low serum iron may suggest iron deficiency, but ferritin and iron studies (TIBC/TSAT) are needed for confirmation. If iron deficiency is confirmed, ferrous bisglycinate is often better tolerated in people with sensitive stomach/IBS-D. Take it once daily or on alternate days as advised, and continue until Hb normalizes and iron stores are replenished. Avoid taking iron with tea/coffee, calcium/dairy, or antacids; separate these by 2–3 hours. Your vitamin D is also mildly low and can be supplemented

Aapki reports dekhne par Hb 10.2 g/dL aur serum iron 37 µg/dL hai, jo iron deficiency ki possibility ko strongly suggest karta hai. Vitamin B12 normal hai. Vitamin D 23.6 ng/mL mildly low/insufficient range mein hai. Meri advice hogi ki iron treatment start karne se pehle serum ferritin, TIBC/transferrin saturation aur CBC indices (MCV, MCH, RDW) ko bhi consider kiya jaye, kyunki sirf serum iron se iron deficiency confirm nahi hoti. Aapka stomach sensitive hai aur IBS-D bhi hai, isliye main unnecessarily high-dose iron se start nahi karunga.
