What you mention about your childhood seems to be normal imaginative thinking appropriate to age. Howsoever, your current problems need to be attended to immediately. Firstly, improve your lifestyle by including regular walk and meditation/pranayaam in your daily routine along with a healthy diet, regular 6-7 hrs of sleep and some recreational activity. Secondly, get assessed by a psychiatrist for any underlying depression or anxiety disorder. He/she may also assess you for any contributory medical illness. Good luck!
