At 65 years of age, a continuous cough along with general weakness should never be ignored, especially when it has been going on for some time. There can be many possible causes, such as: Chest infection (bacterial or viral) Bronchitis or pneumonia Asthma or COPD( most likely in this age and presentation) Tuberculosis (TB) – very important to rule out in elderly patients Heart-related causes Acid reflux (GERD) Anemia or general nutritional deficiency Since she is elderly and underweight (47 kg), proper clinical examination is very important before starting any treatment. What should be done first: A doctor should visit and examine her at home or in clinic and check: Oxygen level (SpO₂) Blood pressure, pulse, temperature Chest examination General weakness and nutritional status Investigations that may be required: The doctor may advise: Chest X-ray Blood tests (CBC, ESR/CRP, sugar levels) Sputum test if cough is productive Possibly ECG if weakness is significant These tests will help identify the exact cause instead of giving only cough syrup, which is not enough. Treatment: Treatment will depend on the cause: If infection → proper antibiotics + supportive medicines If asthma/COPD → inhalers and specific medicines If weakness/anemia → supplements and diet correction If acidity → anti-reflux treatment About home visit: If she is weak or unable to travel, a home visit by a physician is a very good idea. The doctor can: Do full examination Start treatment immediately Advise necessary tests Decide if hospital visit/admission is needed or not Seek urgent medical attention immediately if any of these are present: Breathlessness Fever not settling Chest pain Blood in cough Very poor appetite or extreme weakness Oxygen level below 94% Helpful tips for now: Ensure adequate fluids and light nutritious food Avoid cold drinks, dust, smoke Keep her warm Do not self-medicate or delay evaluation Nebulisation with hot water and vicks in it.

In a 65-year-old, continuous cough with generalized weakness is NOT something to ignore. Common causes include chest infection, pneumonia, uncontrolled asthma/COPD, post-COVID complications, heart-related issues, anemia, or electrolyte imbalance. Elderly patients can worsen quickly if not assessed properly. A home visit by a doctor is appropriate and recommended if she is weak, unable to travel, or coughing continuously.
Next Steps
• Full clinical examination (chest, oxygen level, BP, pulse) • Assessment for infection or breathing difficulty • Start appropriate medicines (not trial-and-error syrups) • Decide if tests or hospital care are required
Health Tips
• Arrange a home visit doctor consultation urgently • If home visit is not immediately available, she should be seen the same day at a clinic or hospital • Avoid self-starting antibiotics or strong cough suppressants ⸻

Arrange a home visit by a physician as soon as possible for full examination, vitals check, chest auscultation, and review of current medicines. The doctor may advise basic tests like chest X-ray, blood tests, oxygen saturation, and ECG based on findings.
